Before you file
If you have a claim or dispute, please notify the office. The team may take photos on the day, but you should take your own photos and keep your own documentation for this claim.
All claims must be filed through this form.
A claim does not pause your bill
Per the rules of the Illinois Commerce Commission, a claim does not constitute a reason to not pay your bill.
Your valuation selection matters here
If the valuation you selected was the standard level of valuation, or limited liability coverage, you may be compensated only by the weight of the damaged part of your shipment — not by the value of your items.
The deadlines, both ways
- Youmust file a claim within 90 days from the date of delivery, or the scheduled date of delivery, for loss or damages.
- Wemust respond within 30 days after receipt of a claim for loss, damage or overcharge.
- Wemust pay, decline, or make a written firm offer — held open for 30 days — to settle with the claimant within 120 days after receipt of the claim, unless an extension of time is agreed for a definite period.
